To Request Release of Records:

Requests for release of records must be received in writing (email, fax, or mail). Please include the name of the decedent, the date of death, and specify exactly what you are requesting (Coroner’s Report, Autopsy Summary, Toxicology).

Electronic release of records is free of charge (email). Paper copies are charged a $0.25 fee per page (fax and email) and fees will need to be paid to the Defiance County Records Center. A typical request including the Coroner’s Report, Autopsy Summary, and Toxicology is usually 2 – 5 pages.

Death Certificates are on file with the Defiance Health Department.
